In providing a quality health service, this practice complies with the Australian Charter of Healthcare Rights. If you feel your rights have been breached, please let us know. We welcome any opportunity to improve our standard of service and uncover any problems. You may add a comment to our suggestion box or make a complaint verbally or in writing, alternatively should you feel unable to do so, you may have the support of an independent advocate from the Office of the Health Ombudsman 133 OHO (133 646) or visit http://www.oho.qld.gov.au. MANAGEMENT OF YOUR PERSONAL HEALTH INFORMATION

Your medical record is a confidential document. It is the policy of this practice to maintain security of personal health information at all times and to ensure that this information is only available to authorised members of staff or funding organisations.

Medicare Card (http://www.humanservices.gov.au/customer/services/medicare/medicare-card)

Required for claiming a Medicare benefit, visiting a doctor who bulk bills, seeking treatment as a public patient in a public hospital or having a Pharmaceutical Benefits Scheme prescription filled.

Concession or Health Care Card

Having a concession or health care card gives you access to cheaper health care services and less expensive medicines in addition to your Medicare Card entitlements. For example, Pharmaceutical Benefits Scheme (PBS) medicines will be cheaper, or free, once you reach the PBS Safety Net threshold. Your partner and children may also be covered by the card if it relates to an income support payment. If you have a Low Income Health Care Card your children may also be covered by the card.

DVA Concession Card (Veteran’s Affairs)

The Department of Veterans’ Affairs also issue their own concession cards to eligible veterans, former Australian Defence Force members and their dependents. A Gold Card entitles the holder to DVA funding for services for all clinically necessary health care needs, and all health conditions, whether they are related to war service or not. Services covered by a DVA White Card are the same as those for a Gold Card but must be for treatment of war caused or service related accepted disabilities. The DVA Orange Card enables the holder to access the range of pharmaceutical items available under the Repatriation Pharmaceutical Benefits Scheme (RPBS). The Orange Card is for pharmaceuticals only, and cannot be used for any medical or other health care treatment.
